You will need Acrobat Reader to open the PDF, fill in the required fields using your computer (except credit card information), and print out the form. Otherwise, just print out the form and fill in by pen. • Enter the required contact / billing / shipping information. • Mark shipping preference: Overnight, 2nd Day Air, 3rd Day or Ground • List the part numbers and quantities you need. • Click here to view our Short Price List in a new browser window, so you can enter the price for each item and please check to make sure you meet our $20.00 minimum order. If you are using Acrobat, the totals will be calculated automatically. Once the form is filled out, you have 3 options: 1. If you filled out the form using Acrobat Reader, "save as" your PDF with the a filename with date and name, ie: "mmddyy_YourLastNameOrCompany.pdf". Email your newly saved PDF as an attachment to microformmodels@msn.com and for security purpose, do not fill in the Credit Card information. Once we have the email in house, we will contact you by phone for your credit card information OR 2. Call us at 508-485-9333 or toll free 877-489-3011, with the form in hand. OR 3. Fax the form to 508-485-9333 or US toll free 877-489-3011. We will call you back to confirm receipt of the order. NOTE: If you choose our PayPal option, make sure you include the email address you want us to send the PayPal "Request for Payment" notice to. Orders can always be taken directly over the phone, but by you having a completed form in hand, it will allow us to process your order more effectively, plus you will have a copy for your records.
